Quilt label?

I’m going to donate a blue jean quilt/throw to be used for a fund-raiser for a h.s. junior girl, a former kindergarten student of mine.  She is waiting for a two lung transplant. 

Should I put a label on this?  If so, I would like suggestions for what  should be put on the label.

    Yes, please put a label on your wonderful quilt. If you can squeeze it all in, your name, the month/year, the recipient's name & the reason why this quilt was made would be great.

    You might not know the recipient if the quilt will be auctioned, but you could put the name of the former student on it.  Maybe something like:  "In support of Jane Doe, August 2006, by Your Name."  Quilts are works of art and love and deserve to be signed and treasured!
